Ayrshire and Arran

Incidents logged:

2009-10: 8,663, of which 50 extreme; 218 major

2010-11: 9,007, of which 84 extreme; 332 major

2011-12: 9,067, of which 94 extreme; 267 major

(Examples: extreme = unexpected death, suicide, homicide, HAI with possible fatal results. Major = wrong patient treated, wrong body part operated on, severe HAI, infant abduction)

 

Borders

Incidents logged:

2009-10: 3,083, of which 7 extreme; 27 major

2010-11: 2,758, of which 14 extreme; 34 major

2011-12: 7,847, of which 13 extreme; 66 major

 

Dumfries and Galloway

Incidents logged:

2009-10: 3,561, of which 2 caused multiple or repeated fatalities or severe financial loss over £2.5m; and 13 caused single deaths, major disability requiring lifelong care, or major financial loss up to £2.5m.

2010-11: 3,229, of which 0 caused multiple or repeated fatalities or severe financial loss over £2.5m; and 15 caused single deaths, major disability requiring lifelong care, or major financial loss up to £2.5m.

2011-12: 3,033, of which 0 caused multiple or repeated fatalities or severe financial loss over £2.5m; and 4 caused single deaths, major disability requiring lifelong care, or major financial loss up to £2.5m.
