An offshore worker was injured when a compressed gas cylinder flew through the air and hit him on the leg.

The incident on Shell's Brent Delta platform in the North Sea happened in November last year.

The firm was issued with a prohibition notice by the Health and Safety Executive (HSE), which said there was not a "safe system of work" at the time for handling and using the cylinders.

Shell said action has since been taken.

A spokeswoman added: "At the time of the incident, the team on the platform took immediate action to bring the situation under control. We have also taken action to address the issues raised in the notice.

"The safety and well-being of employees remains Shell's top priority."
